Local Hospice team members provide care to patients who are typically in their last 6 months of life.  This multidisciplinary team supports the patient and the family.  This team consists of physicians, nurses, aides, social workers, spiritual caregivers, counselors, therapists, and volunteers.

Local Hospice team members work with the patient and the family to develop a personalized care plan.

Local Hospice staff members respect patient wishes and help foster communication amongst family members.

Local Hospice staff are on call 24 hours a day, seven days a week.

Local Hospice staff offer bereavement services to families after their loved on has passed.

Local Hospice team members are specially trained to provide medical assistance and to deal with the loneliness and fears experienced by both the patient and his or her loved ones.
